  • RYLA Delegate Registration - For parents/guardians along with their students to register for the camp. There is no cost to the parents/students.
  • RYLA 2026 Club Sponsorship - For Rotary Clubs to confirm how many students they'll be sending to RYLA, and submit their payments.
  • RYLA Staff Application - For new and returning staff to apply to join us on RYLA staff. RYLA leadership will confirm with all applicants after selections have been made.

RYLA Delegate Registration
(Parents/Guardians Registering Your Students)

 
You will use this form to submit the following:
  • Your contact and personal information (Both student and parent/guardian)
  • Confirmation of your sponsoring Rotary Club
  • Acknowledgement of the Emergency Contact & Consent Form
  • Completion of the Health Form (be prepared to include any medications, allergies, nutritional requirements, other medical limitations/accommodations)
  • Acknowledgement of the Code of Conduct
Please Note:
It is in your best interest to fill this form out completely, accurately, and as soon as possible.
Due to camp capacity limits, availability will be on a first come first served basis. Spots will be held first for students that have submitted this form in addition to their sponsoring clubs completion of all necessary club specific information and payment.
 

Code of Conduct Agreement:
Rotary District 6250 and Upham Woods Outdoor Learning Center provides a positive learning experience for youth. Their health. welfare and positive development is our most import and consideration.

Because youth represent a large number of families from a wide variety of backgrounds and family customs. we want to be sure that we have common expectations. Parent or guardian and youth are to read and discuss the following expectations

  1. Youth should be responsible and sufficiently mature to conduct themselves at all times in an appropriate manner. Youth are expected to respect the rights of others to hear speakers and others during the programs
  2. Youth are to participate in the scheduled activities related to their staff positions while at the camp experience
  3. Youth will abide by the safety and behavior guidelines of Upham Woods Outdoor Learning Center and their school or group
  4. Youth will accept that responsible behavior includes no possession or use of alcohol. tobacco and nonprescription drugs and weapons before. during or after this camp experience.
  5. Youth will not leave Upham Woods without con suiting the teacher or leader in charge.
  6. Youth will abide by the camp policy that no food/ candy, cell phones and radios/music players be brought to camp
  7. Youth will refrain from participating in initiation ceremonies. hazing, harassment. and other behaviors that involve humiliation or embarrassing another person Such activities will not be tolerated.

Consent for Medication Treatment and Medication Administration:
To the parent(s)/legal guardian(s):

If your son, daughter, or ward will be under the age of 18 while at the University of Wisconsin, it is event/camp policy to secure your consent for medication distribution and for the use of medical devices. The medication or medical device can be self administered or be administered by designated camp health staff with the exception of controlled drugs, all medication must remain in the original packaging (bottle la be led with the youth participant's name, doctor's name, medication name, dosage, prescription number, date prescribed, and instructions). A limited amount of medication for life-threatening conditions may be carried by the youth (i.e. EpiPenĀ®, inhaler, etc.). Please note that controlled drugs (Le. codeine, Ritalin, Adderall , Dexedrine, etc.) must, by law, be administered by health staff.

It your son, daughter, or ward will be under the age of 18 years while at the event/camp, it is our policy to secure your consent tor alI of the following. By signing below as parent/guardian,

  • I am giving my consent in advance tor medical treatment at an appropriate medical facility in case of illness or injury.
  • I confirm that I have read the program description and that the youth can participate in planned activities.
  • I am aware of and accept the risk inherent in the program activity.
  • I attest that all information in this registration is accurate.
  • I agree to hold harmless and indemnity Rotary District 6250, the Board of Regents of the University of Wisconsin System, and the University of Wisconsin, their officers, agents, and employees from any and all liability, loss, damages, costs, or expenses which are sustained, incurred or required arising out otthe actions of my son, daughter or ward in the course of the event.
By completing the registration form, you are committing to adhere to the above consent and agreements.
 

RYLA 2026 Club Sponsorship
(Rotary Clubs Submitting Your Club Application & Payment)

 

This form is to be completed by a designated Rotary Club RYLA Contact, or other Rotary Club Leadership. Please be sure that your club has budgeted for the following students to be sent to RYLA and that you are prepared to coordinate their transportation to/from the camp.
 
You will use this form to submit the following:
  • Your contact information
  • The number of students your club will be sponsoring at RYLA
  • The names and gender of those students
Note: You are simply acknowledging the names of the student(s) you're registering for RYLA, and completing payment. Those students, along with their parents/guardians will use the "RYLA Delegate Registration" ticket to sign up their students individually.

RYLA Staff Application
(New and returning staff applying to join the team)

We are so excited for your interest to be on staff at RYLA!
 
This form is intended to be your application to join the staff, as well as your registration so we have all necessary info, health forms, etc. ahead of the event. Completing this form does not guarantee you a staff position but is an important first step. If you are a minor, it is essential that you get necessary permission and information from your parents/guardians to join us at RYLA.
 
Staff training days will be determined at a later date by selected staff and RYLA Executive Team. They will include a virtual training prior to camp and an onsite training session for Leadership Guru's, Junior Counselors and new Rotarians/Rotaractors on the Thursday evening before this year's event.

Staff Training: Thursday evening, May 7th
RYLA Camp: May 8th-10th

APPLICATIONS DUE: March 15th
